Family History Life Insurance Underwriting Guide by Company

American General    Note: family history of cardiovascular disease or cancer prior to age 60 is not a factor for Preferred Plus and Preferred Non-Tobacco, i.e. diagnosis prior to age 60 of these conditions, but did not die, not a factor.

Preferred Plus and Preferred Non-Tobacco: No death from cardiovascular disease or cancer (colon, lung, melanoma, pancreatic) in either parent prior to age 60. No death from breast, ovarian, prostate cancer in parent of same sex prior to age 60.

Standard Plus Non-Tobacco (term only): 1 death from cancer (colon, lung, melanoma, pancreatic) in ether parent prior to age 60. 1 death from breast, ovarian, prostate cancer in parenet of same sex prior to age 60.

Preferred Tobacco:  No death from cancer (colon, lung, melanoma, pancreatic) or cardiovascular disease prior to age 60.  No death from breast, ovarian, prostate cancer in parent of same sex prior to age 60

NOTE: Ignore family history if proposed insured is age 65 or older. Disregard family history if adopted or unknown.  Disregard cervical cancer and non melanoma skin cancer.

American National

Preferred Plus: No death or occurrence CAD, CVA or Familial Cancers (breast, colon, lung, ovarian, prostate or melanoma) in parents or siblings prior to age 65.

Preferred: No death from CAD, CVA, or Familial Cancers (breast, colon, lung, ovarian, prostate or melanoma) in parents or siblings prior to age 60

Standard Plus: No death from CAD or CVA in parents or siblings prior to age 60

Not considered at ages 71 & up

Life Insurance Company of the Southwest (LSW) / National Life:
  Note:  Allows heart disease or cancer, but not death, prior to age 65 for Elite

Elite: No family history (parents or siblings) of death from cardiovascular disease or cancer prior to age 65.

Preferred & Select: No family history (parents or siblings) of death from cardiovascular disease or cancer prior to age 60.

Lincoln National   Note: Cancer not a factor  allows heart disease, but not death, prior to age 60 for Preferred Plus

Preferred Plus Non-Tobacco
Up to age 69: No deaths of parent or sibling prior to age 65 due to cardiovascular disease
Age 70 and up: Disregard family history of cardiovascular disease

Preferred Non-Tobacco
Up to age 69: No deaths of parent or sibling prior to age 60 due to cardiovascular disease
Age 70 and up: Disregard family history of cardiovascular disease

MassMutual     Note:  no reference to cancer

Preferred Best, Preferred, Select: No family deaths from Heart Disease, Hypertension, Cancer or Diabetes under age 50 in parent or siblings; no two deaths under age 60 from same list.

Penn Mutual   

Preferred Plus (permanent products):  No family history of diagnosis or death from coronary artery disease, diabetes or cancer prior to age 60.  Family History will be discounted if Proposed Insured has reached age 60.

Preferred (permanent products): Family history of not more than one cardiovascular or cancer death prior to age 60.  Family History will be discounted if Proposed Insured has reached age 60.

Protective Life

Select Preferred: No death from cancer*, heart disease, or any cardiac-related condition, of either natural parent or sibling prior to age 60. Waived if the applicant is actual age 60 or older unless both natural parents died from one of the same preceding impairments prior to age 60.

Preferred:  No death from cancer*, heart disease, or any cardiac-related condition, of either natural parent or sibling prior to age 60. Waived if the applicant is actual age 60 or older unless both natural parents died from one of the same preceding impairments prior to age 60

*Family history cancers are limited to those types that clearly demonstrate a genetic predisposition, i.e. breast, colon, prostate, ovarian, melanoma, lung cancer.

Non-Tobacco Plus: No more than one death of a parent or sibling before age 60 due to diabetes, heart disease or cerebrovascular disease

SBLI 

Preferred Plus: No diagnosis of parent or sibling, prior to age 60, of cardiovascular or familial cancer. breast, ovarian, prostate, melanoma, thyroid, lymphoma, colon. (Gender specific cancers applicable to same-sex applicants only) Waived if 65 or older and meet all other preferred-plus criteria.

Preferred: No death of parent prior to age 60 or cardiovascular or familial cancer: breast, ovarian, prostate, melanoma, thyroid, lymphoma, colon (Gender specific cancers applicable to same-sex applicants only)  Waived if 65 or older and meet all other preferred-plus criteria.

Family history is disregarded if age 70 or older

Symetra     note: preferred cancer not a factor

Super Preferred Non-Nicotine: No death of parent or sibling prior to age 65 from heart disease, coronary artery disease or cancer

Preferred Non-Nicotine/Standard Plus Non Nicotine/Preferred Nicotine:  No death of parent or sibling prior to age 60 from heart disease or coronary artery disease

Disregard family history if the insured is age 70 and older


Transamerica:

– Includes cardiovascular disease or the following cancers: breast, ovarian, lung, melanoma, prostate and colon
– Some cancers may require evidence of routine surveillance screening

Index UL:

Preferred Elite: No family deaths before age 65 of either parent or sibling.
Disregard if proposed insured  is age 65 or older.
